The Dr. Jane Frederick Family Foundation IVF Grant was created to expand access to fertility care by helping qualifying patients offset the cost of treatment at HRC Fertility.
Through this partnership, selected recipients may receive financial assistance toward fertility treatment, including HRC Fertility’s Promise Package.

The HRC Fertility Promise Package is a comprehensive IVF program designed to provide greater financial predictability throughout the fertility journey.

The program is designed to help patients pursue treatment with greater confidence by bundling essential IVF services into a single package. Eligibility requirements apply and treatment plans are determined by your physician.
Applicants may be eligible if they:

A medical evaluation completed by your fertility specialist outlining your diagnosis, treatment plan, and estimated treatment costs.
A written statement sharing your family-building journey and explaining how grant assistance would impact your path to parenthood.
Complete the Baby Quest Foundation application.
Submit all required documentation before the application deadline.
Applications are reviewed by Baby Quest Foundation and grant partners.
Selected recipients are notified following the review period.
Grant funds are applied directly toward treatment at HRC Fertility.
