The purpose of your initial consultation at HRC Fertility (HRC) is to evaluate your medical history and discuss a personalized treatment plan for you. Our goal is to make you an informed participant in your treatment and to provide expert reproductive care in a compassionate setting.
Please plan on staying 1.5 to 2.5 hours with us, including wait time, as you will be spending quality time with both your physician and nurse coordinator. Because we feel it is extremely important to obtain as much information as possible, we encourage and invite both partners to attend the initial consultation if applicable. In your first consultation, you will have one-on-one time with your doctor, followed by a baseline ultrasound, and also one-on-one time with your nurse coordinator.
In order to assist your doctor, it is preferred that you submit medical records or lab results in advance so there is adequate time to review your medical history in detail. If needed, we can also provide you with a medical records request.
HRC will email you a welcome packet to bring with you on the day of your consultation. We include a patient questionnaire that you will need to complete prior to your appointment. You can also prepare questions for your doctor prior to your appointment using the You and Your Doctor form.
Prior to initiating treatment, a financial consultation will need to be scheduled. This meeting is designed to provide you with information regarding HRC’s financial programs, and if applicable, to understand your insurance coverage. Learn more about our financial programs.
In order to verify your insurance benefits, please provide us with a fax copy of your insurance card, or you can provide the information over the phone. We are currently contracted with most PPO plans and select HMO groups. If you have an HMO, an authorization must be submitted prior to your appointment.
When you meet with your financial advisor, he or she will discuss with you the financial packages associated with your treatment plan. We have financial plans that meet almost every budget.
We are open every day, but we are open on the weekends only for routine appointments and procedures.
